- This form is used to determine clubs that will be accepting new members create promotional items and share contact information for new and prospective families.
End-of-Year Club Report Forms
Due Jan 31
- Club/Group Activities Form-This form ensures all your club meetings and activities are compliant with insurance requirements.
- 4-H Club/Affiliate Yearly Financial Summary - This form is required to file IRS tax documents on your clubs behaf
- 4-H Club/Affiliate Equipment Inventory Form -This form is required by Ohio 4-H and OSU if your club owns any equipment purchased with 4-H club funds.
- Club Audit ( You will be contacted if you are required to turn this in. Each club will be audited every 3 years.) Form used for audits below. Please use to conduct internal audits when necessary.
Enrollment/ Entry Forms
Due April 15
- Ohio 4-H Club Charter Checklist (on file at county office) (revised 10.2022)
- Club Constitution (on file at county office) (revised 4.2023)
- Bylaws Optional Template (on file at county office) (revised 4.2023)
- *Club member list (please write out name LEGIBLY next to signature)
Enrollment Checks will also be due April 15th $15.00 a youth capped at $45.00 a family.
SPIN (Special Interest Club) Forms
These forms are due before the start of the SPIN Club's meetings for the year. If the club will be held more than once per year the member list and Activity Form must be turned in before each offering. Club Charter may be turned in once per year by April 15th.
